Essential fields in any CPD record
- Activity title — Identifies the entry when searching your portfolio later
- Activity type — Categorises the learning (course, audit, teaching, etc.)
- Provider — Shows the accrediting or delivering organisation
- Date completed — Confirms the activity falls within the revalidation period
- Duration — Hours and minutes — used to calculate CPD points
- CPD points — The credit value (auto-calculated or from a certificate)
- Category — Clinical / Academic / Management-Leadership / Personal — required by most bodies
- Learning environment — Internal / External / Personal
- Description — A brief summary of what the activity involved
- Reflection — What you learned and how it affects your practice
- Evidence reference — Certificate filename, publication DOI, or meeting minutes ref

If you prefer a paper record, each major UK regulatory body publishes its own template:
